Miss Black Georgia & South Carolina USA Small Business Sponsor/Partner Registration

Greetings!

We are excited to announce the 2026/2027 Miss Black Georgia & South Carolina USA Pageants! Thank you for your interest in being a sponsor and/or partner.

Please complete this form to secure your sponsorship/partnership.

Then pay the corresponding sponsor fee to confirm your registration. Please understand that the sponsorship/partnership fee and all additional fees paid to The Miss Black Georgia and/or South Carolina USA Pageant(s) and/or A. Van Dyke Productions are non-refundable and non-transferrable.

I hereby grant A. Van Dyke Productions and/or The Miss Black Georgia and/or South Carolina USA Pageant(s) to release my likeness to the local media outlets, allowed by A. Van Dyke Productions and/or The Miss Black Georgia and/or South Carolina USA Pageant(s), to identify the appropriate media and release the information about my participation as a sponsor.

1. understand that my participation as a sponsor/vendor shall refer to any travel, workshop/curricula, or event within the A. Van Dyke Productions and/or The Miss Black Georgia and/or South Carolina USA Pageant,

2. do hereby understand the nature of my participation as a sponsor/vendor, and I release A. Van Dyke Productions and/or The Miss Black Georgia and/or South Carolina USA Pageant to include all its administrators, members, directors, agents, officers, volunteers, sponsors, partners, affiliates, and/or employees from all liability for any unintentional injuries sustained during my participation as a sponsor/vendor.

3. do consent to emergency medical treatment in the event of such an emergency, given that emergency contacts cannot be reached,

4. understand that A. Van Dyke Productions and/or The Miss Black Georgia and/or South Carolina USA Pageant, or its affiliates does not provide insurance for sponsors/participants/vendors and any bill incurred due to emergency treatment will be my responsibility. I also certify that I am covered by insurance that will cover any treatment caused by injury and if not that I am solely responsible for payment of any bill incurred due to emergency treatment and that myself and/or my insurance company only are responsible for payment of such treatment.

5. and that I certify that the above information is true and accurate.

I acknowledge and understand that there is a risk of injury involved in various situations. I agree to follow the rules and the instructions of A. Van Dyke Productions and/or The Miss Black Georgia and/or South Carolina USA Pageant to reduce the risk of injury to myself and others. However, I acknowledge and understand that A. Van Dyke Productions and/or The Miss Black Georgia and/or South Carolina USA Pageant cannot eliminate the risk of general injury. Injuries may and do occur. Injuries can be severe and, in some cases, may result in permanent disability or even death. I freely, knowingly, and willfully accept and assume the risk of injury that might occur from participation in the A. Van Dyke Productions and/or The Miss Black Georgia and/or South Carolina USA Pageant.

By selecting “I Understand & Agree” below, I agree that I have fully read, understand, and agree to the conditions set forth below, and submit such choice as a binding signature of agreement:

I acknowledge and agree that, during the 2026/2027 Miss Black Georgia and/or South Carolina USA Pageant and any corresponding events hosted by A. Van Dyke Productions, I may be subject to all rules and procedures set forth by the A. Van Dyke Productions and/or The Miss Black Georgia and/or South Carolina USA Pageant. I acknowledge and agree that I am responsible for informing appropriate personnel of any medical needs and authorize A. Van Dyke Productions and/or The Miss Black Georgia and/or South Carolina USA Pageants' personnel, or other event officials, to take all medical actions they believe necessary until such time as a family or otherwise be contacted. I acknowledge and agree that A. Van Dyke Productions and/or The Miss Black Georgia and/or South Carolina USA Pageant, its affiliates, officers, employees, agents, and volunteers do not have or assume any liability for damages, losses, or injuries I may encounter as a result of my participating as a sponsor for A. Van Dyke Productions and/or The Miss Black Georgia and/or South Carolina USA Pageant. I acknowledge and agree that any revocation of permission given by this form can only be in writing and must be delivered to the appropriate event officials to be effective.
